Payment - payment is expected at the time of your visit for services not covered by your insurance. We will attempt to identify these in advance for you. Any outstanding balances will be billed directly to you and should be paid immediately upon notification. New Patients - If you are a new Summit patient then we have created a package of documents that we need you to complete prior to your first visit. You can read and complete at your leisure. Insurance Companies - We accept most insurance plans; however, please call us to confirm your plan will be accepted. Please bring your health plan insurance card with you to the office for each appointment. Our staff will update our records to ensure correct processing with your insurance company. Payment may be required if insurance eligibility cannot be verified.
