We accept most insurance plans. However, please check with us ahead of
time to confirm your plan will be accepted.

Please bring your health plan insurance card with you to the office for each
appointment.  Our staff will update our records to ensure correct processing
with your insurance company. Payment may be required if insurance eligibility
cannot be verified.

Payment is expected at the time of your visit for services not covered by your
insurance. We will attempt to identify these in advance for you. Any
outstanding balances will be billed directly to you and should be paid
immediately upon notification. Payment for services may be paid by
credit/check card, or check.

If you are not sure how your insurance company works, we have provide links
to all the major companies above to help you.
