First Time Patients - If you are currently not a
patient and will be making an appointment with us, it
would be very helpful if you complete the following
steps prior to coming to our Office: New Patient
Registration Package .

Insurance Companies - We accept most insurance
plans. However, please check with us ahead of time to
confirm your plan will be accepted.

Please bring your health plan insurance card with you to
the office for each appointment.  Our staff will update
our records to ensure correct processing with your
insurance company. Payment may be required if
insurance eligibility cannot be verified.
Newsletters - We will be publishing Newsletters periodically to tell you about
our new services, procedures and offerings.  Also, we will provide you health
tips and interesting facts. Our newsletters can always be found on the
following page, so you may want to bookmark the page - [ Newsletters ].
Web-Sites - There are many medical reference web-sites available to help
you understand almost any medical condition.  We are providing you links to
the ones we think stand out above the rest  [ Medical Web-Sites ].
Pharmacies - We have located all the pharmacies near our Office and have
provided links and maps for you [ Pharmacies near our Office ].
Payment - payment is expected at the time of your visit for services not covered
by your insurance. We will attempt to identify these in advance for you. Any
outstanding balances will be billed directly to you and should be paid
immediately upon notification. Payment for services must be paid by cash or
check.  We do not accept credit cards.
